The GP Out of Hours based at Macclesfield Hospital is looking to recruit a Band 6 Triage Practitioner who will work on a part time basis mostly over the weekend period but with the potential for some weekday evenings or nights across a 7 day roster. The GP Out of Hours service provides Urgent healthcare and advice during Nights, Evenings, Weekends and Bank Holidays when patients own doctors surgeries are closed and we work closely with NHS 111 Service and NWAS (North West Ambulance Service) in providing this care. As one of our Triage Practitioners, you will clinically assess and prioritise all patients who are referred to the GP OOHs service by 111, NWAS, ED and other HCP's (Healthcare Professionals). You will assess patients presenting symptoms and decide whether face to face treatment is required, either urgently or at a later date, signpost to the appropriate place of care or provide self care advice appropriate to the patients needs. You will be a Registered Nurse or Paramedic with 2 years post registration experience. Hours per week: 15:00 x 2 posts Main duties of the job To provide clinical triage as an integral part of the GP Out of Hours Service for the population of Eastern Cheshire CCG (Clinical Commissioning Group). To work autonomously but as part of a team of Triage Practitioners, GP's, Call Handlers and Drivers within the GP Out of Hours service based at Macclesfield District General Hospital. The post holder will utilise clinical triage and assessment skills, whilst working within the Scope of Professional Practice and adhering to local policies/guidelines. The post holder will manage and prioritise direct referrals to the service, from ED, NWAS and other HCP's with a professional and courteous manner at all times.
