Main duties of the job
Working remotely, our team triage information from colleagues and members of the public in the Wolverhampton locality to ensure that we refer and signpost people to the most appropriate service to meet their needs. In summary, our Care Coordination work streams include:
1. Triage over the telephone or via video consultation to support people with an acute need to avoid admission.
2. Triage for patients with long term conditions requiring ongoing health and/or therapy support while residing in their own homes.
3. Offer a see and treat face to face triage service for patients who have an unmet need.
As a Junior Sister/Charge Nurse within the Care Coordination, you will be required to provide managerial leadership support to the team, support staff in their development and lead with service innovation. Experience and confidence in utilising computers and web-based applications is essential.
