In accordance with the practice rota, as agreed, the post-holder will make themself available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly referring patients to other services/agencies in a timely manner, utilising Practice, PCN and local guidelines Recognise the need for and be able to perform and interpret investigatory procedures, including laboratory and radiology. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Ensure appropriate follow up of patients Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation Assess the health care needs of patients with undifferentiated and undiagnosed problems Screen patients for disease risk factors and early signs of illness In consultation with patients and in line with current practice disease management protocols, develop care plans for health Admit or discharge patients to and from the caseload and refer to other care providers as appropriate Record clear and contemporaneous consultation notes to agreed standards Collect data for audit purposes To be involved in the provision of education, supervision and training to GP Registrars and other staff Compile and issue computer-generated acute and repeat prescriptions Prescribe in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate Be aware of your duties and responsibilities regarding current legislation and adhere to our policies and procedures on Safeguarding Children and Safeguarding Adults The post-holder will be required to visit patients during surgery time in an emergency if clinically indicated The post-holder may need to undertake On Call sessions and participate in the Enhanced Access Service The post-holder will undertake fair share of home visiting sessions per week as allocated
