We are looking for an enthusiastic band 5 community staff nurse to work with the OOH District nursing service 3 nights per week. You will join a team of nurses delivering high quality care to patients who are housebound.
You will be caring for patients with long term conditions and complex health needs, developing your skills and knowledge, whilst working towards an integrated healthcare structure. You should be able to demonstrate you can make clinical decisions to provide evidence-based practice and care planning in response to clinical assessments.
Main duties of the job
The successful candidate will perform holistic assessments of patient nursing needs. Plan, assess, implement and evaluate care delivery according to changing health needs utilising prescribing skills.
Ensure practice is evidence-based through undertaking audit and critically appraising literature to achieve a high standard of care.
The post holder actively supports the District Nursing team by undertaking responsibility for a delegated caseload.
Liaise with the District Nurse team leader to ensure information sharing in relation to patient care, changes to care plan, and the continuity of care and reviewing of complex care packages.
Mentoring, Teaching and Assessing.
Liaises with all health care professionals and other agencies to ensure effective service delivery.
Involve patients and carers in the planning and delivery of care.
Maintain accurate records in accordance with NMC Standards.
To participate in the health needs assessment of the practice population to identify areas of unmet need.
Ability to work without direct supervision and to solve problems.
Work on a rota basis to facilitate weekend working across the district nursing teams.

Duties and Responsibilities:
To promote the health and well-being within the family unit and community as a whole.
Perform comprehensive assessment of patient nursing needs as the named nurse.
Plan, assess and evaluate programmes of care according to changing health needs under the direction of the District Nurse.
To support the team leader in the co-ordination of complex and continuing care packages.
Maintain accurate records in accordance with NMC.
Attend a multi-disciplinary case conference as the named nurse if appropriate.
To communicate and collaborate with other members of the PHCT, voluntary and statutory organisations to support individual patient care.
To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
To competently perform clinical skills related to the post.
To be professionally responsible for a delegated caseload under the supervision of the District Nurse.
To assess health needs at individual, practice and community level.
To undertake chronic disease management reviews using appropriate template.
To work without direct supervision.

Skills & Attributes
* Able to perform complex assessments, plan, implement and evaluate care packages.
* Wide range of clinical skills.

* Knowledge of professional and NHS issues relating to specialist knowledge.
* Ability to develop effective interpersonal relationships with colleagues across health and social care settings.

* Supra pubic catheterisation.
* Tissue viability qualification.
* IV Therapy.
