Hazelwood House is a 12 bedded and a 1 bedded bespoke pod, low secure unit for adults with a primary diagnosis of Learning disability and secondary mental health issues. We have a strong focus on recovery and enabling patients to reintegrate back into the community.
We are part of the Priory group and you will work closely with other clinicians in the region based at Lincoln, Mansfield & Nottingham.
Key Responsibilities
1. Lead a multidisciplinary team of professionals from nursing, clinical psychology, and occupational therapy.
2. Review treatment plans and coordinate discharge.
3. Liaise with Commissioners regarding admission, progress updates, and discharge planning.
4. Participate in a multidisciplinary approach to assessment, formulation, and management plans.
5. Provide evidence of GMC registration when requested by the Hospital Director or Chief Medical Officer.
6. Work autonomously to agreed short and medium term objectives.
7. Utilize problem-solving skills and systemic knowledge to translate best practices into operational solutions.
8. Participate in the non-residential medical on-call rota.
About You
You must possess highly sophisticated communication and interpersonal skills as these are critical to gain the trust and build the confidence of patients, in order for treatments and interventions to be effective and successful. You must be a good problem solver and be able to use your systemic knowledge, experience and creative thinking.
