Job Overview
A unique opportunity has arisen to apply for the newly created role of Discharge Support Worker within our Community Hospital Services to be based on Charnwood Ward at Loughborough Community Hospital.
The post holder will be required to have experience of working within a hospital setting and knowledge of the patient cohort as the main part of the role will be supporting our patients through their discharge journey and maintaining patient flow.
You will provide clinical and administrative support to a multi-professional team so knowledge of roles within the MDT, the voluntary and private sector is essential. The post holder will also be required to complete discharge paperwork, generate letters, utilise IT for record keeping and participate in audits.
This is a really interesting and important post which is crucial to ensuring the safe and timely transfer of our patients from our services to their next place of care or home.

The working hours for the post will be 08.00 - 16.00 or 09.00 - 17.00 Monday to Friday.

Job Description Summary (Main duties and responsibilities)
1. To develop close working relationships with ward teams, providing clinical and administrative support to the multi-professional team relating to the discharge process, having delegated responsibility under the direction of an appropriate registered healthcare professional.
2. Support the delivery of effective patient discharges from the ward, assisting with processes/procedures to enhance patient flow as identified by the multi-professional team.
3. Undertake specified and supervised aspects of discharge planning as delegated by the registered healthcare professional within an agreed protocol, sphere of responsibility and after completing appropriate competency training agreed by the trust.
4. To provide next day telephone follow-up as appropriate for patients discharged and take actions and/or escalate issues as required.
