We are building a Primary Care-based Mental Health & Well-being Team.
We have an opportunity for Band 5 Primary Care Practitioners (ARRS) who are Registered Professionals to work in our local Primary Care place-based team in North Warwickshire.
This is an exciting opportunity for dynamic people to join us and build on the existing multi-disciplinary PCN (Primary Care Network) teams to support the mental health and well-being needs of local communities.
You will be employed by Coventry and Warwickshire Partnership Trust but be embedded within the North Adren Primary Care Network.
This role is directly patient-facing, so you will have a meaningful impact on improving the access, experience, and outcomes for service users on a daily basis.
These local teams will work in partnership with higher acuity mental health services (delivered in this area by Coventry and Warwickshire Partnership Trust), offering care at the point of need which suits individual patient needs. Each team will work together with support from their practices to ensure they are delivering the best care they can.

Your responsibilities will include:
1. Offering brief interventions to people who have mental health needs, but do not need a referral into SCMH services. This will include facilitating short-term case management, offering professional advice, joint support, and information.
2. Working closely with social prescribers and other professionals within the PCN multi-disciplinary team to support the patient in accessing the right services in the right place and at the right time.
3. Acting as a liaison worker between Secondary Care Mental Health Provider and Primary Care, supporting clinical decisions and discussions for patients who are not known to SCMH services or those who are open to Community mental health services.
4. Reviewing patients in primary care and assessing where their needs can be best met, and supporting discussions on patients who have been discharged or not engaged with SCMH services.
5. Providing an opportunity to support primary care and alleviate suffering and distress in a timely way for patients, enhancing patient journeys and creating better joint working across primary care and adult secondary care mental health systems.
6. Developing and maintaining collaborative links with the third sector to provide clinical advice and support to Mental Health social prescribers or equivalents within Primary Care, including providing supervision of clinical cases as required.
Essential Criteria:
* A good standard of education including minimum GCSE Grade C and above in English and Maths (or equivalent).
* Educated to degree level or equivalent experience.
* Registered professional qualification (RMN/Social Worker/Occupational Therapist).
* Evidence of an active interest in further professional development.
* Professional clinical knowledge acquired through degree/diploma.
* Relevant experience working with individuals or professional groups.
* Knowledge and experience in Mental Health services, including needs and interventions required for routine referrals and a theoretical knowledge of this.
