Main duties of the job
NHS NW London Hounslow Place Team is looking for a dynamic and enthusiastic colleague to take the lead on developing integrated neighbourhood teams across the Borough. They will also be the local lead on primary care and will work in close collaboration with the full range of statutory and non-statutory partners. The ideal candidate will have current and demonstrable knowledge of the national agenda for integrated neighbourhood teams and primary care and be able to demonstrate how they will use their understanding to make an impact in Hounslow. A thorough understanding of population health management approaches will be needed to help drive the improvements needed across the Borough to reduce inequalities and deliver the ICB priorities. Experience of delivering complex transformational projects, including working with a diverse range of stakeholders to achieve the desired outcome will be essential along with good project management and problem solving skills.
Job responsibilities
1. Lead identified complex and strategic projects and programmes across NW London that are within the team portfolio, that may include the need for public engagement or consultations within the work plan.
2. Establish a clear programme of work and set of deliverables, articulating outcomes and benefits that will be delivered (clinical and financial), timescales for delivery and resources required.
3. Articulate the transformation staffing requirement to deliver each priority and work with leadership team to identify suitable people to work on the programmes.
4. Manage projects and programmes to deliver to these agreed outcomes and timescales.
5. Ensure programmes of work are centred around reducing inequalities and agree clear, measurable metrics to assess progress.
6. Ensure delivery of the NW London finance schemes associated with this programme.
7. Ensure that the programme has robust management structures and processes including: delivery plan, risk and issue management, governance, reporting, change management, dependency management.
8. Ensure regular programme board meetings are held and that these are effectively constituted to perform their role; receiving appropriate and timely information to do so.
9. Support local areas and providers in local implementation of models of care to agreed NW London principles.
10. Ensure proactive and effective links with enabling work streams including Business Intelligence, workforce and communications.
11. Through effective outcomes management ensure programme are delivering to constitutional standards.
12. Strong communication skills and relationships management required to enable delivery of complex work programmes working with internal and external partners including where divergent views and resistance to change.
13. Promote a culture that embraces equality and strives continuously to reduce inequality in both access and outcomes for all NW London residents.
14. Lead on strategic plans for designated areas - developing in partnership with residents and other stakeholders.
15. Have a good working knowledge of NHS Standard Contract.
16. Where relevant, work with the Contracting Team, to translate strategic intentions and agreed care pathway changes, into robust measures written into contracts.
17. Where relevant, work strategically with the wider contracting team across NWL to ensure that the priorities articulated within the NWL Strategy, and any other national or local priorities, are embedded within contracts appropriately.
18. Be a source of knowledge to support local leads and partners, enabling best practice, ideas and issues are learned across the system.
19. Utilisation of professional experience in delivering impactful change for patients to ensure system approach to transformation and commissioning.
20. Budget holder for a defined budget where responsibility has been delegated including supporting related contract management and performance oversight.
21. Provide leadership and day to oversight of local primary care relationships, development of improved primary care access and local enhanced services.

Experience
* Experience of delivering complex transformational projects, including working with a diverse range of stakeholders to achieve the desired outcome.
Financial and Physical Resources
* Previously responsible for a budget, involved in budget setting and working knowledge of financial processes.
Qualifications
* Educated to masters level or equivalent or equivalent level of experience of working at a senior level.
* Evidence of continuous professional development.
