We have an exciting opportunity to join a newly formed and innovating Intensive Support Team (IST). We are looking for enthusiastic and person-centred senior registered nurses to join our team.
The multi-disciplinary IST is highly skilled, passionate and completely person-centred, comprising of Nursing, Occupational Therapy, Psychology and Speech & Language Therapy. The team's overall aim is to provide dynamic support to people living in the community and prevent an admission to hospital / placement breakdown.
The team work across Durham, Tees Valley to support people with learning disabilities who have experienced physical and emotional trauma and may have additional mental health needs, sensory processing difficulties and/or communication needs.
The team has 3 workstreams Home Support, Wrap around and Provider. This advert is for the wrap around support stream (please note that this is a community post but there is an expectation that all staff will be trained to PAT Level 2 - restraint trained to support the wraparound pathway - please consider before applying). Staff working in our wrap around stream will have the opportunity to work with people with complex needs supporting individuals and the MDT to bring complex packages of care to life, meet people's needs in the right way, improve their quality of life and prevent admission to hospital.
Main duties of the job
This role requires 7 day working between the hours of 8am and 8pm.
Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
The successful applicant needs significant experience in working with adults with learning disabilities, mental health difficulties and/or complex needs.
The bases are within both Durham and Teesside and can be negotiated on appointment to the team.

As a senior registered nurse in the team, you will support a small caseload of people and the MDT around them. You will also support a dedicated wrap around clinical nurse specialist in the development of wrap around plans. The wrap around plan will be implemented by the person's care team with the support and practice leadership of the IST.

1. Current professional registration with Nursing & Midwifery Council - RNMH or RNLD (RNSC - CAMHS only ) - RGN (If appropriate)
2. Evidence of Continuing Professional Development
3. Recognised Practice Supervisor and/or Practice Assessor, clinical educator or equivalent. Must be achieved within 6 months
4. Trained in Clinical Supervision and providing clinical supervision OR willingness to work towards within 6 months
Knowledge
Essential
5. Understanding of relevant legislation (. Mental Health Act, Mental Capacity Act).
6. Detailed understanding of Safeguarding and its application in practice
7. Clinical Risk Assessment and Management and its application in practice.
8. Clinical Governance and its application in practice

10. Understanding of psychological models of care and treatment
11. Working knowledge of clinical medicines management, including administration of injections
