CHOC primary care network is looking to recruit a forward thinking and enthusiastic social prescribing link worker to join our established social prescribing team. Our PCN area covers practices across Congleton and Holmes Chapel (CHOC) Primary Care Network. We are seeking to appoint a flexible minded social prescriber to take a front line role working with the practices. Social prescribing empowers people to take control of their health and wellbeing through referral to link workers who give time, focus on what matters to me and take a holistic approach to an individuals health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support. This role will help a range of patients, some with long term conditions manage their health differently and more effectively or help people who are lonely or isolated, or who have complex social needs which affect their wellbeing, including their mental health. You should be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing complex social and emotional challenges. Main duties of the job Manage a caseload of patient referrals that meet the criteria for the social prescribing service Build trust and respect with individuals, providing non-judgemental and non-discriminatory support Work with individuals to produce a personalised support plan to address their health and wellbeing needs based on the their priorities, interests, values, cultural and religious/faith needs and motivations to improve their health and wellbeing Support patients with their personalised support plan to provide continued support towards achieving their goals Liaise with health professionals and seek advice and support to discuss patient-related concerns Promote social prescribing Build relationships with staff in GP practices within the PCN Develop links with all local agencies in line with the social prescribing implementation plan Assess, monitor and manage risk including suicidal ideation and safeguarding issues. Educate staff within the PCN on what services are available within the community Forge links with local VCSE organisations, community and neighbourhood level groups Work to identify unmet diverse needs within the community and gaps in community provision Work with people, their families and carers Produce basic reports and analysis. By working together with our local providers of care we are improving the patient experiences of primary care. We are in the Cheshire East Council geographic area.
