This Co-ordinator role organises our carers to ensure they provide effective, safe, individual care to our service users. What the job involves:

Responding to emails and telephone calls in an effective and timely manner, liaising with the Registered Manager to ensure all carers are following agreed care plans

Maintaining full and complete knowledge of the service users: their care needs; care plans; assessments; schedule of care; regular carers
*
Maintaining up to date care plans, risk assessments and service user details
*
Have full knowledge and understanding of CQC regulations and Company policies and procedures and to highlight any issues arising to the Registered Manager
*
Attending meetings with the management team, service users, their families and/or representatives to ensure that the service provided meet the current service user’s needs

Co-ordinate with local community teams...
