The Bank Palliative Care Specialist Nurse (CNS) will join an established Specialist Palliative Team which provides a 7-day service.
The post holder will be expected to carry out a comprehensive holistic assessment of need for patients with a palliative diagnosis, using their clinical reasoning and expert knowledge to ensure the delivery of quality clinical outcomes while working in close collaboration with other health and social care professionals.
The post holder will contribute to the wider palliative and end of life care agenda within the community in Luton, provide education across a range of professionals, and ensure the team provides a flexible responsive service that meets the local needs within Luton.

Salary will be subject to the 2024/25 Afc increases once an implementation date is agreed.
Main Duties of the Job
1. Provide leadership, clinical excellence, and innovation in palliative and end of life care within the Luton community nursing service.
2. Work in partnership with other providers to deliver a seamless service across all care settings.
3. Provide support and be proactive in specialist education and training to other professionals involved in palliative and end of life care in the community setting.
4. Act as a role model for palliative and end of life care within the organisation demonstrating high standards of care and providing clinical leadership to others.
5. Review local and national strategy for palliative and end of life care and support local implementation of changes.
Job Responsibilities
1. Undertake holistic health assessments of patients with complex palliative and end of life care needs in community settings.
2. Proactively plan, co-ordinate, deliver and evaluate the provision of high-quality specialist palliative and end of life care within the community and across the primary and secondary care interface ensuring onward referrals are made as appropriate.
3. Make appropriate clinical decisions independently and seek senior guidance where necessary in reviewing, monitoring, and managing patients.
4. Assess, plan, and evaluate care for patients, their families, and carers.
5. Monitor patient needs and interventions and appropriately report to the identified key worker.
6. Provide a therapeutic relationship with patients with a palliative diagnosis, their carers, and their families.
7. Provide opportunistic informal education to patients, their families, and health professionals.
8. Provide patients and their families/carers with information.
9. Work as part of a multidisciplinary team acting as patient advocate for specialist palliative and end of life nursing advice and care.
10. Actively participate in the Palliative and End of Life Care educational programme to deliver evidence-based education to community-based staff.
11. Maintain personal development, knowledge, and skills sharing good practice with colleagues.
12. Actively participate in regular clinical supervision.
13. Develop and sustain effective working partnerships with local clinicians, other specialist palliative care providers, managers, and team colleagues.
14. Understand and ensure good practice in managing patient information between different care settings and teams.
15. Develop and maintain good working relationships with key resources that support service development and influence the role e.g., Macmillan Cancer Support.
16. Observe and maintain strict confidentiality regarding any patient/family/staff records and information in line with the requirements of the Data Protection Act.
Person Specification
Qualifications
* RGN
* Educated to a degree or evidence of working at level.
* Palliative care/oncology postgraduate course
* Undertaken or willingness to undertake extended nurse prescribing course.
* Evidence of recognised communication skills training (ACST)
* Non-Medical Prescribing
* Psychological skills training
Experience
* Currently working in B6 role or above.
* Significant and recent clinical knowledge of palliative and end of life care
* Evidence of teaching/training experience.
* Audit/evaluation of service/project.
* Experience of multi-professional working
* Current knowledge of national policies and drivers relevant to specialist palliative and end of life care.
* Community Nursing experience

* Ability to demonstrate good IT skills.
* Excellent communication and interpersonal skills - written and verbal with evidence of basic counselling skills.
* Ability to be flexible working across organisations and work with a range of professionals.
