Who we are looking for
We are seeking a compassionate and trauma-informed professional to join our Post Care Support Service (PCSS) team. As an Intake and Brief Support Facilitator, you will be the initial point of contact for enquiries and referrals, conducting eligibility and risk screenings and verifying out-of-home care experiences. You will provide specialised intake services and promote a 'no wrong door' approach by facilitating referrals for clients with complex issues to appropriate RASA services or external support.
In this role, you will ensure continuity of support by assisting with client liaison and collaborating with other RASA staff and programs. You will also provide short-term, time-limited interventions, such as supporting clients with grant applications and monitoring those in the step-down stage of their support journey.
To be successful in this role you will need:
* Cert IV (AQF level 4) in Counselling, Community Services, Youth work, or Mental Health with demonstrated relevant experience and skills.
* Experience working with Aboriginal and Torres Strait Islander people, families or communities effectively, using culturally responsive approaches and processes.
* Experience working with complex client matters.
* Experience in delivering trauma informed practice.
* Experience in holistic screening, assessment, referral and advocacy, case management and crisis work in the community services sector.
It would be a bonus if you have:
Experience working with individuals dealing with the impact of trauma caused by abuse, violence, childhood trauma, neglect and crime.
What you will be doing
* Provide holistic, trauma-informed and responsive frontline triage, intake, support, and referral to clients seeking to access PCSS.
* Identify and respond to immediate client needs and safety issues in consultation with the Regional Coordinator, Senior Support Facilitators, or Manager.
* Facilitate access to services through intra- and inter-agency referral, liaison, and provision of case management support.
* Contribute to providing disability-accessible and culturally responsive services.
* Work collaboratively as a member of the PCSS team to ensure effective ongoing client engagement, case coordination, and referrals.
