Job summary An exciting opportunity has arisen for a Band 6 physiotherapist to join the Integrated Independence Team in the Home Treatment and Reablement service on a fixed term basis to cover a secondment. The Integrated Independence Team (IIT) is a dynamic and committed service that provides multidisciplinary care to service users in Hackney. The IIT consists of many subteams and expertise across the teams, including Home Rehabilitation and Reablement, Rapid Response, Virtual Frailty Ward and Discharge to Assess. The team consists of Physiotherapists, Occupational Therapists, Speech & Language Therapists, Social Workers and Assistants. This post would suit an individual wishing to develop their clinical skills in community based rehabilitation and intermediate care, with an interest in frailty, falls and elderly care.
