* Responsible to: Senior OT Practitioner
Responsible for: The D2A Occupational Therapists will use their OT expertise to assess and recommend appropriate discharge pathways and care needs of service users, with a focus on driving recovery. They will also enable them to achieve maximum independence in their own homes, within the context of the relevant legislation and departmental responsibilities. They will assist the manager with agreed delegated responsibilities in the delivery of effective management and development of the Reablement and Independence Service. Number of reports: Direct reports: 0 Indirect reports: 0 Total Managed: 0 Financial Responsibilities: None - however the emphasis on all workers has altered over time and a key part of the role is to identify ways to support individuals and their carers through strength based conversations and reduce overall expenditure.,
* To work alongside the Hospital Social Work team and Reablement and Independence Service
* Use Occupational therapy expertise to assess, implement, evaluate and review plans for service users and carers, enabling them to achieve maximum independence in their own homes.
* Provide a broad range of support to clients who have physical or mental health problems, learning difficulties, dependencies or who are elderly and guide care transfer along the most appropriate discharge pathways.
* Help to develop and/or maintain resident's independence using a strengths based approach.
* Requirement to work on a shift basis including occasional weekends if required.
MAIN ACCOUNTIBILITES:
* The postholder will identify the needs of the service users and carers within the context of their own homes and within available resource.
* To involve carers in this process and take into account their needs as well as those of the service users' to ensure best outcomes.
* To manage autonomously a clinical caseload of clients at a specialist level, assessing and maintaining clinical records.
* Participate in clinical supervision and appraisals, and supervise OT Assistants according to level of expertise.
* To ensure that all activities are informed by up to date manual handling advice and training.
* To set SMART goals to ensure treatment is effective, and to improve client flow through the service and along appropriate discharge pathways including in-reach.
* To work collaboratively with health and social care community and acute colleagues, including the Hospital Social Work Team and the Reablement and Independence Service.
* To work with other members of the MDT to ensure effective assessment, treatment, management of care pathways and transfers of care / discharges.
* To support the development of specialist programmes of assessment, treatment and intervention for clients within a community setting, this includes in-reach work for those in residential placements.
* To be responsible for the issue and safe use of equipment used in your area of work and to adhere to the appropriate service policies.
* To participate in the team CPD activities, supervision process and peer review, maintaining an up to date CPD portfolio This should demonstrate responsibility of maintaining own competency to practice.
* To carry out other tasks as are commensurate with the post according to OT core skills.
This role will require you to use your car on work related business every day. It is your responsibility to ensure that you are insured to do this in additional to having the general business use insurance cover.
On occasion you may be required to transport equipment/or other staff members in your car to carry out your duties. Prior to doing this a full risk assessment should be carried out.
