Key responsibilities include:
1. Sourcing and securing appropriate care home placements.
2. Negotiating cost-effective care packages while maintaining high-quality standards.
3. Liaising with care providers and stakeholders to ensure seamless transitions.
4. Ensuring compliance with CQC guidelines and safeguarding policies.
What we're looking for:
1. Experience in adult social care, specifically brokering for 65+ residential and nursing care.
2. Strong negotiation and organizational skills.
3. Knowledge of relevant systems (e.g., Mosaic, Framework-I) and legislation (Care Act 2014, DoLS).
4. A compassionate, person-centered approach to care coordination.
