Job summary Based in East Kent, the Urgent Care Service / Frailty Home Treatment Service is an innovative team working across various services and locations. We are a blended service withdiverse clinical backgrounds working together for the benefit of frail patients. We are seeking Advanced Practitioners (AP) with NHSE digital badges or those willing to work towards this qualification in a trainee capacity. Candidates will complete an MSc in Advanced Clinical Practice as apprentices, developing their clinical portfolio over three years. We have two positions available, with a preference for those already holding APdigital badges and appropriate clinical experience. The AP role is undertaken by experienced registered health or social care practitioners, characterized by complex decision-making and a high degree of autonomy. Advanced practice posts include 10% non-clinical time to contribute to all four pillars of advanced practice and are underpinned by a master's level award or NHS England accreditation.APs will function effectively across the four pillars of advanced practice: clinical practice, leadership and management, education and training, and research, quality improvement,and service development. We deliver services seven days a week, 8am-8pm, across various locations. Candidates must be able to drive and have access to a vehicle, oftenworking alone. Main duties of the job East Kent community services Our diverse, talented team includes frailty consultants, a consultant advanced practitioner (AP), GPs, consultants-in-training, and trainee APs. Wesupport activities including:Home Treatment Service People prefer receiving care closer to home. During the pandemic, we established a service to meet this need. The seven-day service offers expertfrailty advice across East Kent. We aim to provide alternatives to hospital admission for those in frailty crisis. Near-patient testing and hospital-at-home capabilities offer robustalternatives to ED attendance, treating over 200 patients with COVID and hypoxia.Community Hospitals We support five hospitals in East Kent, offering step-up/step-down services, complex assessments, rehabilitation, and end-of-life care. Learning from theCOVID pandemic, we provide intravenous therapies as an alternative to acute hospital admission. A review of bed use will help us develop community treatment units withincommunity hospitals. Primary Care Network (PCN) Hubs Each PCN holds virtual multi-disciplinary team meetings for complex case management. Our participation identifies those who would benefit from a CGA or care planning. Proactive Comprehensive Geriatric Assessment (CGA) We provide domiciliary assessments for frail individuals at risk of crisis, incorporating CGA, and anticipatory and advance care planning.
