The role will include providing a clinical pharmacy service to general surgery patients: offering advice to healthcare staff, patients and carers on all aspects of the safe and effective use of medicines supporting ED and Pre - Operative Clinic working collaboratively with community services to support discharged patients supporting initiatives relating to quality of care providing specialist support in MDT meetings providing timely medicine reconciliation and prescribing of medicines to aid the flow of patients and transfer of medicines across the interface offering prescribing support to optimise patients' medicines and expedite discharge. Main duties of the job To be a source of expert specialist advice for matters relating to Surgery to other pharmacists and other healthcare professionals To act as an independent prescriber in a specialist area To be a role model for other pharmacists to lead, plan and develop an advanced specialist clinical pharmacy service To be responsible for delivering the defined service Key Performance Indicators and include close working with other Pharmacists. To deliver specialist pharmaceutical care including non-medical prescribing to a defined caseload of patients, in line with the agreed scope of practice, undertaking risk management and ensuring compliance with local and national clinical guidance. To support the Clinical Service and Pharmacy department in meeting its risk management, financial management and clinical governance responsibilities around the use of medicines. To lead and develop clinical pharmacy audit and practice research for paediatric patients. To take an active part in projects to support patient safety.
