An exciting opportunity has arisen for a new and innovative role as an Assistant Practitioner, Band 4, to work alongside the Breast Clinical Nurse Specialist Team based at Ysbyty Gwynedd and Llandudno post holder will work as part of a designated Breast Team, delivering evidence based high quality clinical care that focuses on the direct needs of the patient. The post holder will work under the direction and support of a Breast Cancer CNS to implement the Cancer Delivery Plan for Assistant Practitioner will manage a small defined caseload of patients under the supervision of the CNS and ensure effective communication with patients, carers, multidisciplinary team and the wider health and social care community. They will work proactively as a member of the multi disciplinary.
The ability to speak Welsh is essential for this post.
Main duties of the job
The Assistant Practitioner will work alongside the Cancer Clinical Nurse Specialist (CCNS) to improve the experience of care for people with cancer. They will work within the multidisciplinary team providing support, dealing with non-complex tasks to allow registered practitioners to focus their expertise on managing patients complex care needs. They will promote self-management and provide coordination of care, utilising a person centred approach from diagnosis, through treatment and beyond.
The Assistant Practitioner will carry out interventions within agreed parameters and defined protocols of care under the delegation of registered nurses within the speciality.

Qualifications
Essential
1. To be educated to NVQ level 3 and be prepared to work towards diploma / level 4 or equivalent within 2 years to provide the theoretical knowledge as well as having the practical skills.
Desirable
2. Knowledge and understanding of Cancer and its consequences.
3. Evidence of CPD
4. Recognised communication qualification within 3 years.
5. Knowledge of local policies/procedures and guidelines.
Experience
Essential
6. Significant experience of work within a health care setting.
7. Experience and able to utilise all Microsoft packages including Excel, SharePoint, WPAS & WCP
8. Previous relevant experience in a healthcare environment working in an indirect supervised role within an agreed scope of practice.
Desirable
9. Experience of working with patient complex needs resulting from a Cancer diagnosis.
10. Experience of working at a higher level as a healthcare support worker.
11. Experience of monitoring long term conditions.
12. Experience of educating patients to manage their own condition.
