Responsibilities :
* Conducting comprehensive assessments of adults with disabilities, identifying needs, strengths, and risks.
* Developing and implementing personalized, innovative care plans that promote independence, well-being, and transformation.
* Providing ongoing support and intervention to adults with disabilities and their families, ensuring access to necessary services and resources.
* Collaborating with healthcare professionals, community organizations, and other relevant agencies to coordinate comprehensive and transformative care.
* Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in line with legal and professional standards.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
