Job summary
We are lookingto recruit a Community Nurse who has enthusiasm, compassion and commitment tosupporting and enabling people with Learning Disabilities to join our wellestablished Community Learning Disability Team across South Tyneside.
You will workwithin the multi-disciplinary team and provide Community nursing support toadults with Learning Disabilities. You will also work with all stakeholders, tocontribute to learning and development of staff working with adults withLearning Disabilities, ensuring effective service delivery and the bestpossible outcomes for our service users
You must haveup to date knowledge and appropriate skills which will ensure that CommunityNursing interventions are delivered in the way which meets the needs of theservice user.
You will havean understanding of the complex needs of people with learning disabilities andbe able to demonstrate this with recent and on-going clinical professionaldevelopment. You must have excellent communication and organisation skills. Youmust also have experience of working within a Community Learning DisabilitiesTeam.
You will managea case load and supervise junior staff with a delegated caseload and supportthem in their development.
You will be acar driver and have access to a vehicle to meet the requirements of this post.
Main duties of the job
1. To provide an assessment and therapeutic service to Adults with a Learning Disability who have a range of complex needs in a variety of settings including day services, residential settings and the persons own home; assessing, prioritising, formulating, planning, treating and discharging patients autonomously.
2. To work as a member of the Community Learning Disabilities Team, contributing to multi-disciplinary working as necessary, to address the needs of clients referred to the team and manage a delegated caseload.
3. To work collaboratively with other specialist services within Learning Disabilities to meet the needs of clients.
4. Be regularly involved in supporting and consulting on complex transitions to new home environments; leaving school; admission and discharge from inpatient services.
5. To participate in Service Developments, Audit and R&D activities as directed by the Appropriate Lead.
6. To actively participate in a programme of supervision, competency development and CPD activities under the guidance of senior clinicians. To work within guidance and framework for Safeguarding Adults / Child Protection Policies.
7. To provide verbal / written reports in respect of individual clients.
8. Work within clinical governance framework.
9. To promote a healthy lifestyle within the client population.
10. Attend multi-disciplinary reviews and CPAs.
11. Act as care coordinator.
