Job Title Acute Admissions Unit Clinical Fellow Location London Facility Central London - Victoria Institute Acute Admissions Unit AAU-Cleveland Clinic London Job Code UK0221 Job Summary This is a role for an Acute Admissions Unit (AAU) Clinical Fellow who will support a multidisciplinary team, being responsible for examination, investigation, diagnosis, and treatment of acute medical problems. Through close collaboration with all clinical institutes, this role will help to deliver high quality patient care in a safe environment and assist with decision making in complex medical case management. This will take place in two settings: the AAU and through a medical consultation on a specialty ward within the hospital. The Department Chair of Medicine, the AAU Clinical Fellows, and 10 other medical consultants (1FTE), as well as the AAU multidisciplinary team (Pharmacy, Nursing, Junior Staff, Physiotherapy/Occupational Therapy, and Care Management) will provide: • Acute assessment for new medical admissions • Ward cover for general medical inpatients • Acute medical consultation service for inpatients • ICU step down patient’s liaison • Daily “Hot Clinic” for rapid post-discharge follow up (within the same week or next 5-7 days) to maximize continuity of care The AAU will not be directly responsible for the management of: • Elective surgical patients • Patients accepted by specialty teams with known acuity and diagnosis, who will be admitted directly to speciality wards Job Details Job Responsibilities: Clinical Working as a part of the AAU clinical fellow workforce covering the Acute Admissions Unit (AAU). Coverage over days and nights in line with the European Working Time Directive. During the day, the fellow would work closely with the lead clinician. At night, the fellow would be working independently under supervision. Being a part of the rolling rota covering a 24-7 acute service. The 12 bed AAU is embedded within the critical care floor with a further 29 ICU beds. The ICU is staffed 24/7 with on site Consultant and clinical fellow cover. It is anticipated that the AAU and ICU fellows will work collaboratively to optimize care standards in the AAU setting. For those interested in critical care provision this will offer further educational and clinical support for the fellow and the AAU Lead and assist with admission of new and inpatient consultation during the weekend and bank holidays as required, including morning weekend post take MDT ward rounds with team Shared oversight with the Department Chair over all AAU admissions, selecting and directing timely transfer to appropriate specialties for onward treatments, and medical consultations on surgical wards or the ITU Participation in daily ward rounds and/or MDT board round, for General Medical base ward patients Identification of patients who qualify for rapid follow up in the outpatient setting for patients discharged within the same week (Daily Hot Clinic) per policy guidance Professional Participating in the regular in-house learning sessions and other educational activities as well as taking up educational responsibilities Adhering to the best practice guidelines, clinical audits, and governance. Active involvement in clinical governance meetings, including but not limited to mortality review, safety event reporting, peer review, and other relevant activities. Continual commitment, and participation in mandatory trainings, including Epic electronic medical record system. As detailed above a 4 track educational program shared across the UK and US campus is in place. Fellows will elect which track the wish to enter alongside clinical roles. It is expected that as part of the educational mission of CCL, fellows will participate in in professional development opportunities such as medical management & leadership, education & simulation or academic research programmes within CCL or the main campus in Cleveland, US. A key component of this educational program will be a visit to main campus in Ohio, integrated into the chosen educational or leadership track. I nterdependencies The AAU is highly dependent on: Close collaboration with reservations, the ICU, and the acute surgical wards Close communication and timely collaboration with multidisciplinary specialist expertise across all clinical institutes Adoption of CCL policies, best practices, and guidelines, assuring appropriate response times and smooth transfer to specialist care Rapid onsite diagnostic capabilities and reporting Robust care management and discharge planning Certifications: MBBS or equivalent Fully registered with the GMC and current license to practice Current UK visa or permission to work ALS certificate Competencies (Complexity of Work): Ability to organise and prioritise complex demands Demonstrates awareness of the basics of managing medical patients, including emergencies Effective communication skills Evidence of skills in the management of acute medical emergencies (e.g. ALERT, IMPACT certification) ( desirable) Information technology skills using electronic patient care systems ( desirable) Evidence of working in a multi-disciplinary team ( desirable) Competent in routine clinical skills including venipuncture, blood cultures, arterial sampling, nasogastric tube and urethral catheters.
