Job summary An exciting opportunity has arisen for a Clinical Nurse Lead within Brentwood & Billericay Community Health Team, this is an exciting and autonomous role supported by the District Nurse Team Leads and Operational Lead. Working in a clinical role across the borough the successful candidate will review and implement plans of care for complex patients across the borough, acting as a clinical expert and role model to the Community Nursing and Mental Health and Well being teams Main duties of the job The post holder will demonstrate excellent communication and negotiation skills, as well as a sound knowledge base linked to advanced physical assessment and treatment planning of individuals with complex comorbidities that require a multidisciplinary approach from the Primary Care Networks(PCN). The Clinical Nurse Lead will be required to support the community nurses and the PCN partners to coordinate and deliver the care to this cohort of complex patients, ensuring that services wrap around and support them to maintain their health and wellbeing in their normal place of residence. The post holder will be a Registered Nurse (RN12-NMC) and will be required to support the Community nursing teams and the Primary Care Networks(PCNs) to coordinate and deliver the care to patients within the respective localities, including being professionally and legally accountable. About us Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.
