Job summary Dual Diagnosis Practitioner We are seeking a Dual Diagnosis Practitioner to join our HMR Living Well Teams. In this role, you will support a defined caseload of clients facing both mental health challenges and substance use issues, whether or not they have received a formal diagnosis. You will work across the care pathway and into specialist services, assisting individuals with complex needs that include current or historical substance use from various sources such as legal, illicit, or even online avenues. In addition to direct client support, you will act as a specialist resource for other staff members working with this client group. Your expertise in assessment, management, and intervention will be vital in guiding your colleagues and ensuring comprehensive care is provided. About us Pennine Care NHS Foundation Trust is excited to advertise a permanent 37.5 Hour post for a Senior Mental Health Practitioner to work in the local Heywood, Middleton and Rochdale Primary Care Network (PCN). This role supports the wider transformation of community mental health services for adults and older adults that seeks to bridge the divide between primary care and secondary mental health care, and physical health services. The Senior Mental Health Practitioner will be employed by Pennine Care but based within the HMR Primary Care Network, within GP surgeries and will be accessible and responsive to GPs and service users within the Primary Care network.
