We have an exciting opportunity for a Clinical Lead to spearhead our Transfer of Care Hub at Northwick Park Hospital.

As the Transfer of Care Hub Clinical Lead, you'll provide expert clinical and operational leadership in collaboration with the Harrow IICCS team, CLCH Head of Clinical Services, and our partner organisations. Your role is crucial in delivering safe, timely discharges across multiple pathways ultimately improving patient outcomes and experiences.
What You'll Do
1. Lead & Advocate: Serve as a professional practitioner and ambassador for community services, championing CLCH's commitment to excellent care.
2. Coordinate Discharges: Work hand-in-hand with acute hospital teams to ensure patients receive prompt, efficient, and high-quality transitions from hospital to home.
3. Drive Improvement: Collaborate with the IICCS Service Manager and LNWH Head of Discharge Operations to cultivate an environment of continuous quality improvement, making sure our services evolve to meet patient needs.
4. Support & Empower: Provide effective professional leadership for all relevant pathways, fostering a culture of learning, development, and proactive resource management within the team.
Main duties of the job
About You
You're a forward-thinking clinical leader with experience in discharge planning or intermediate care coordination. You excel at collaborating across multidisciplinary teams and thrive in dynamic settings where patient-centered care is paramount. The ideal candidate should be a strategic thinker capable of developing strategic service improvements and managing complex patient care pathways. You must possess strong communication skills, with the ability to manage relationships with a diverse range of internal and external stakeholders effectively. Additionally, we are looking for dedicated healthcare registered professionals who have a first-level degree and/or significant equivalent experience in managing discharge and patient flow, coupled with a passion for community healthcare.

Job description
Job responsibilities
1. Develop detailed and complex technical understanding of discharge to assess pathways and hub processes to lead service delivery from a point of professional expertise and competence.
2. Work with the Integrated Intermediate Care Coordination Service (ICCS) Manager in assuring safe and effective service delivery including responsibility for data reporting.
3. To deputise for the Integrated Intermediate Care Coordination Service (ICCS) Manager where necessary including attendance at required system or NWL forums.
4. Demonstrate patient-centered values ensuring that all patients are treated with compassion and dignity.

Qualifications
Essential
* First level degree or equivalent
* Evidence of further relevant courses
* Equivalent significant experience of managing discharge and patient flow
* HCPC Registered Therapist / First Level Registered Nurse or Therapist currently registered with relevant professional body

Experience
Essential
* Has the required level of knowledge of area of specialty and be able to apply that knowledge in order to promote the health of the patients.
* Extensive experience at minimum of Band 7 (Agenda for Change) or equivalent - minimum 3 years
* Extensive experience of senior management
* Experience in team management including performance management
* Experience of receiving and delivering supervision
* Able to teach in formal and informal settings
* Budget Management
* Proven ability to prioritise and manage own workload with minimal supervision
* Experience of initiating and implementing changes to services
* Experience of working collaboratively with a variety of agencies and professionals and across organisational boundaries.
* Evidence of innovation in practice evaluation and audit skills.
Desirable
* Demonstrates high level skills to communicate effectively and appropriately with partners, patients, staff and stakeholders
