KEY RESPONSIBILITIES: THE POST HOLDER WILL: PATIENT/ CLIENT CARE To visit housebound patients living in the Borough of Newham; assist in the management of their diabetes care and liaise with the multidisciplinary team as appropriate Be a resource for housebound patients and their families by providing information on all aspects of diabetes care to enable patients to manage their own disease effectively. Maintain accurate, contemporaneous and legible patient records. Work closely with the multidisciplinary team in planning, implementing and delivering programmes of care and to ensure that the objectives of these are met. Assist housebound patients and their carers to achieve individualised goals to maintain their physical and psychological well being in order to become proactive in their own care. Work within East London Foundation Trust policies with regards to extended and supplementary nurse prescribing. POLICY/SERVICE DEVELOPMENT / IMPLEMENTATION Always practice in accordance with East London Foundation Trust Nursing strategies, local and national clinical guidelines, local formulary recommendations and prescribing guidance. Support and participate in Trust strategies for clinical governance to ensure the provision of high standards of care and clinical effectiveness. Be responsible for meeting targets and achieving the trust objectivesregarding clinical governance. Always act in accordance with The Nursing and Midwifery Councils (NMC) Code and Professional Conduct and Scope of Professional Practice. Maintain a high professional standard of care and expertise. Act as an expert resource, providing clinical advice and support to community, hospital based staff and junior members of the team. As part of the nursing team, provide cover for other diabetes specialist nurses. In conjunction with the lead nurse and centre manager, deal with any day- today unit management issues within the diabetes service. Participate in the development and review, where necessary, of all policies, protocols, guidelines and standards of care for the service. Ensure that the nursing aspects of the diabetes service quality improvementplans are implemented. Promote a seamless service between primary and secondary care. Be aware of the national and local policies that affect the nursing and the speciality services, and respond to them. Keep accurate records, submitting statistical returns and compiling reports as necessary and when required. If required, investigate and submit reports on any untoward incident involving patient or staff. Undertake additional negotiated roles/projects or responsibilities, such as health promotion initiatives etc. Participate in planning, organising and undertaking of other activities within the framework of the post, commensurate with the grade. Liaise and work collaboratively with the DSNs in other East London districts,including the multidisciplinary team and NUHT to integrate the service. FINANCIAL AND PHYSICAL RESOURCES Work within the East London Mental Health Trust allocated budget withreference to Nursing Prescribing. Ensure that services are delivered as effectively and efficiently as possiblewithin available resources. Human Resources Identify the training needs for professionals in the primary and secondary care setting and develop and provide appropriate training programmes to meet their needs. Provide clinical expertise and supervision for nursing students in the primary and secondary care setting. Take responsibility for maintaining own clinical expertise in relation to current research and evidence based practice in diabetes care. Participate in formal and informal teaching sessions within the community, including DESMOND Structured Education (Group Programme). Information Resources Keep and maintain EMIS records for every patient treated. Use emails to enhance communication within and between primary care and secondary care. Attend regular multidisciplinary meetings, plan agenda items and takes minutes of the formal meetings. Research and Development Service Development Identify areas for, and participate in, nursing and multidisciplinary research. In agreement with the lead nurse, identify areas for, and actively participate in audit. Develop and facilitate research-based practice within the diabetes nursing team. Develop and initiate new techniques in the care with patients with diabetes,working within the guidelines of National Service Framework(s), Clinical Governance, NICE Guidelines and the NMC contract etc. Provide for the dissemination of current information and theories necessary for the advancement of patient care. Participate in the development and auditing of research based nursing and multidisciplinary standards e.g. Essence of Care. Participate within the diabetes service for implementing and auditing the Essence of Care framework. With the lead nurse and the other members of the team, contribute to the development of agreed protocols, integrated care pathways and audit tools that monitor practicePersonal Development Take responsibility for maintaining all clinical expertise in relation to current research and evidence based practice in diabetes care. Actively participate in appraisal and personal development using reflective practice. Take responsibility for own professional development in accordance with PREP and attend study, course in agreement with line manager. Maintain current registration of the required qualification for the post within the NMC. Adhere to the NMC Code of Conduct. Continue to develop his / her scope of professional practice and contribute to the advancement of the diabetes specialist nursing by attending national and local clinical meetings and feeding back appropriate information. Actively participate in regular clinical and operational supervision. Receive direct patient referrals and be able to refer patients to other services such as podiatry, psychotherapy and dietetics.
