About the service:
Their new service in Tranent is a core and cluster model. There are 9 properties and person centred support will be provided to 10 adults with a range of support needs due to learning and physical disabilities, mental health and autism. Support will enable people to live in their own home on a long-term basis. They work in the East Lothian Health and Social Care Partnership to deliver person centred support whilst using innovative technology to evidence and help manage the service demands.
Overview of the Role:
As a Support Practitioner, you will support people to live the life they choose. By following each person's unique support plan, which details the specific personal outcomes that they're aiming to achieve together, you will recognise and build on people's strengths to encourage positive risk-taking - helping foster choice and control.
Your holistic approach and excellent interpersonal skills will mean that you provide practical support and encouragement across all aspects of people's everyday lives, including:
* Money and budgets
* Health and wellbeing
* Communication
* Planning for the future
* Participation in sports and leisure activities
* Any other aspect of life that is recorded within someone's support plan
Your empathetic nature will understand that we're all unique individuals needing support with different things at different points in our lives.
In this new supported living service, you will:
* Assist individuals with daily living activities within their home environment and in the community.
* Foster a comfortable, safe, and nurturing living space that promotes independence.
* Support residents in planning for their future and reducing social isolation and loneliness.
* Provide personalised care tailored to each resident's needs, including support for Mental Health, Autism, and Physical & Learning Disabilities.
* Encourage residents to participate in household tasks, recreational activities, and community involvement.
What We Offer:
* Competitive Salary: £24,051 - £25,494 / £12.30 - £13.04 per hour
* Overtime Pay: Time and a half payment for hours worked over 37.5 hours per week (averaged over the month).
* Anti-Social Hours Payment: Up to £1209 for waking night shifts (11pm-7am).
