The post holder will be responsible for the provision of occupational therapy within the mental health in-reach service at HMP High Down, as part of a multi-disciplinary team. This will include working with people presenting with complex needs, and challenging behaviour, within the context of a prison environment. This will include contributing to a group programme and people on an individual basis.
The post will involve delivery of the OT provision to a designated caseload, focusing on daily living function. This will include the assessment, treatment and evaluation of clinical interventions, planning and running a range of therapeutic interventions, carrying out functional assessments and contributing to multidisciplinary team care planning and risk assessment. This will include providing specialist input for those accessing support within a dedicated Neurodiverse pathway.

1. Clinical Responsibilities
· To be responsible for the Occupational Therapy assessment, treatment and evaluation of an allocated clinical caseload.
· To be responsible for planning and providing group and individual assessment and treatment sessions within the service.
· To ensure, where appropriate, that handovers are received from colleagues before starting work in designated areas.
· To carry out standardised assessments and provide detailed feedback to the multidisciplinary team about clients’ abilities and also information about future plans and requirements for progress.
· To undertake treatment planning, involving the client in all possible occasions.
· To prompt and motivate the clients to attend and participate in Occupational Therapy treatment sessions.
· To communicate treatment plans and clinical reasoning for interventions to other members of the multidisciplinary team and to work in collaboration with other disciplines and staff in clinical work.
· To plan, run and evaluate treatment sessions on a group or individual basis, using graded activities to achieve treatment aims.
· To maintain a flexible approach to sessions, modifying activities according to the fluctuating clinical presentation of individual clients and the needs of the service.
· To engage with clients with challenging behaviours, contributing to team discussion regarding the clinical approach to individuals and communicating this clearly to all therapy staff working with the client. This may include aggressive and violent behaviour
· To work in collaboration with other members of the multidisciplinary team to provide a coordinated care package.
· To actively contribute to the multidisciplinary teams risk assessment process and obtain agreement from the team before engaging clients in any activity that may require therapeutic risk taking.
· To regularly review each client’s progress in order to evaluate the effectiveness of treatment interventions and modify treatment plans accordingly.
· To ensure OT assessment and treatment is based on best available evidence of effectiveness; using a variety of methods to inform best practice such as: Critical appraisal of literature and research, professional guidelines, outcome measures and clinical audit.
· To ensure that the OT package of care is being followed for all clients they are responsible q for including those of whom they supervise.
2. Communication and Information Management
· The post-holder will have access to sensitive multi-agency information and must be aware of potential public and media interest that this may generate, and deal with these issues in line with Trust policies regarding confidentiality.
· To prepare accurate written reports following assessment in line with quality standards.
· Maintain accurate and up to date electronic patient records ensuring confidentiality is kept at all times.
· Aid in the development of effective working relationships with other service lines and other providers for both statutory and non-statutory agencies
· Maintain links with partnership agencies, which might include Commissioners of services, voluntary organisations and statutory organisations such as Social Services and the police.
· To maintain up-to-date electronic records in line with local policy.
3. Service Development
· Make suggestions and contribute to the formulation of new strategies aimed at improving the effectiveness and efficiency of the service.
· Promote sound clinical practice by contributing to the promotion of current developments in the care of young people with complex needs and challenging behaviour.
· Contribute to the appraisal and audit of own clinical practice and that of others.
4. Knowledge and Education
· To provide input into training sessions for multi-agency staff as part of the in-service training programme and make clinical presentations in team meetings.
· To present and represent the OT department to the wider service and to raise the profile of OT through formal and informal education across the prison estate.
· To attend regular supervision sessions
· To attend mandatory training courses and refreshers as required.
· To attend relevant courses / study days outside the Service and to share information and acquired skills learned with the OT staff / multi-disciplinary team
· To work with sessional teachers in the unit providing support and advice as appropriate.
5. Governance and Performance
· Participate in clinical supervision, appraisal and objective setting.
· To meet with the team to monitor and review the performance, governance, clinical practice and business of the service.
· Provide clinical cover for colleagues in their absence when required.
· Report all sickness, absence, accidents or incidents using the correct Trust processes.
· Keep up to date with Trust and Unit policies/procedures
· Manage time and resources to provide a safe and effective high quality of care.
6. Professional Responsibilities
· To pursue own continuous professional development and to record this through a CPD portfolio
· To participate in internal and external training, in keeping with personal development plan to develop relevant skills and knowledge.
· To be a reflective practitioner, evaluating the effectiveness of own work and learning from experience, including proactive use of supervision and appraisal process.
· To regularly update own clinical knowledge through training, clinical discussion and literature review
· To enhance own understanding of the evidence, base for OT interventions.
· To work in accordance with professional guidelines issued by the College of Occupational Therapy which are relevant to the field of Forensic Psychiatry.
· To promote a positive and professional identity for Occupational Therapy within the field of prison healthcare and Forensic Psychiatry.
· To be involved in available research opportunities with support from the Lead OT.
· To be aware of limitations of own clinical knowledge and seek advice as necessary.
· To maintain State Registration with Health Professions Council.
7. Effort, Mental and Environment
· Able to maintain professionalism in the face of challenging behaviour and in intense environments.
· Able to sustain mental effort for prolonged periods.
