Based at Avalon House, Winchester the Older Adults CMHT offers assessment, diagnosis, treatment and support for people with a deterioration in cognition and/or dementia and over 65 who present with functional mental health illnesses. We work closely with carers and other services, such as Adult Health and Social Care and with the voluntary sector.
The CMHT is a lively, dynamic and supportive multi-disciplinary team which provides care coordination and recovery focused care for individuals with complex and challenging mental health needs. We will ask for a high level of commitment, motivation and knowledge working with people with severe and enduring mental health problems and/or dementia.

Applicants must have a driving licence suitable for use in the United Kingdom and access to a vehicle for work purposes. Our working hours are Monday to Friday 9.00 to 17.00.

Main duties, tasks & skills required
You will need excellent assessment and communication skills and you will carry a caseload and report to the multidisciplinary team. As a clinician, you will need to work alongside other agencies to promote holistic recovery focused care.
You will need to be able to work autonomously in the community and use your own initiative. The successful candidate will have sound clinical skills, be able to coordinate complex care, have good administration and IT skills, as well as good report writing, organisational skills and working knowledge of CPA processes and safeguarding.
You will assess and plan the care for our service users throughout their journey with the service, including assessing risks, develop management plans, review and adjust in line with their needs. Your approach will be innovative and responsive to the individual service user and their circumstances.
You will receive and participate in caseload supervision and management, to ensure that care plans are matched to need.
You are able to prioritise care, understand the principles of Safeguarding and report concerns to your team manager.
You will stay up to date with statutory training, and show interest in continued professional development. You support the team with supervising students.
