Are you a dedicated Band 6 Community Psychiatric Nurse (CPN) or Social Worker looking to make a meaningful difference? A great client of ours is seeking a professional for an Adult Community Mental Health Team, supporting individuals aged 18 to 65 in the Reading area. They offer a flexible work environment with a balance of on-site and remote working.
Role Overview
You will be responsible for managing a caseload of up to 30 service users. You will play a vital role in care coordination, delivering high-quality support and interventions tailored to each individual’s needs.
Key Responsibilities
* Manage a caseload of up to 30 people with diverse mental health needs
* Provide care coordination and case management within the community
* Deliver person-centered assessments, interventions, and care plans
* Work collaboratively with other healthcare and social care professionals
* Attend team meetings and case discussions
* Engage with clients and their families to promote recovery and wellbeing
Requirements
* Valid NMC registration for CPN or SWE registration for Social Workers, with no restrictions
* Experience in community mental health, ideally in an adult setting
* Ability to manage a caseload efficiently, maintaining a high standard of care
* A car and valid driver’s license to travel within the community
* Strong interpersonal and communication skills
