Job summary Primary Care Mental Health Support Service (PCMHSS) Working within the legislative framework of part One of the Mental Health (Wales) Measure, to improve access to mental health support at a primary care level for people with mild to moderate or stable severe mental health needs within the context of a multidisciplinary multi-agency Primary Care Mental Health Support Service (PCMHSS). The service is delivered to working age adults, older adults and people with a learning disability. Main duties of the job An exciting opportunity has arisen for a dynamic and forward-thinking professional with experience of working with adults to join the Monmouthshire Primary Care Mental Health Support Service (PCMHSS). Applicants must be a Registered Mental Health Professional (Occupational Therapist, Registered Nurse - Mental Health, Registered Nurse - Learning Disabilities or Social Worker). The successful applicant will support the wellbeing of adults by delivering the five functions of Part One the Mental Health Measure: o Conducting assessments of mental health needo Providing brief, evidence-based therapeutic interventionso Offering consultation to professionals working at a 'Tier One' levelo Providing guidance and advice to adults and their familieso Supporting onward referral where necessary. Ensuring effective service provision to adults is a high priority within the Gwent PCMHSS. There will be training and developmental opportunities for the successful applicants, as well as the opportunity to contribute to new initiatives and developments within the service.
