As a Care Coordinator, you will play a vital role in supporting both staff and patients, by coordinating and supporting their care across the NHS services. This role involves working closely with general practice teams (including other PCN professionals) within the PCN to help support patient caseloads ensuring their needs are met. You will assist them to manage their health by accessing the necessary services. Additionally, you will handle administrative tasks such as appointment management and for the recording of meeting minutes ensuring all discussions and decisions are accurately documented. The goal is to deliver personalised care, improve patients' quality of life, and contribute to a seamless healthcare experience. This non-clinical role requires a caring, dedicated, and proactive individual with strong organisational and communication skills.
Main duties of the job
Patient Care Coordination: Develop and review personalised care plans with patients, families, and carers. Identify patients needing additional support, particularly those with complex needs.
Assist with appointments, information-sharing, and patient queries to empower informed care choices. Collaborate closely with Primary Care colleagues for cohesive care.
Multidisciplinary Team (MDT) Support: Participate in MDT meetings to coordinate care for at-risk patients. Act as a communication hub, ensuring that all involved professionals are aligned. Refer patients to appropriate services and support networks.
Administrative Support: Handle key administrative tasks, from recording meeting minutes to managing patient records. Support with meeting coordination and data management.
Patient Advocacy and Support: Guide patients in managing their health, accessing educational resources, and participating in shared decision-making.

1. Patient Care Coordination (Primary Responsibility):
o Develop and review personalised care and support plans with patients, families, and carers.
o Proactively identify patients needing coordination, especially those with complex health needs, ensuring they receive appropriate support.
o Manage appointments, address queries, and provide information to help patients make informed choices.
o Collaborate with GPs, social prescribing link workers, health and wellbeing coaches, and other professionals to ensure cohesive care.
o Facilitate communication and follow-ups between providers to support smooth transitions between care services.
2. Multidisciplinary Team (MDT) Support (Primary Responsibility):
o Participate in MDT meetings to coordinate care for vulnerable adults and those at risk of emergency hospital admissions.
o Serve as a central contact, ensuring care plans are communicated to all relevant professionals.
o Refer patients to social prescribing services and other healthcare professionals as needed.
3. Administrative Support (Secondary Responsibility):
o Perform general administrative tasks as assigned by the PCN Manager to support PCN operations.
o Record meeting minutes, capturing key discussions, decisions, and action points for timely distribution.
o Manage patient records with attention to accuracy, confidentiality, and data protection compliance.
o Organise meetings, prepare agendas, and coordinate with multiple stakeholders.
4. Patient Support and Advocacy:
o Support patients in understanding and managing their conditions through tailored information and guidance.
o Help patients access self-management education, peer support, and personal health budgets where applicable.
o Encourage shared decision-making and help patients prepare for clinical discussions.
o Monitor patient progress and outcomes, maintaining accurate records of interventions and their impacts.
