Job summary An exciting opportunity has arisen within the Gloucestershire Continuing Healthcare (CHC) Team for a Complex Case Manager (Band 7) registered professional. The role is planned to build on a combination of Complex Case Management skills & subject matter expertise, and to develop and support a growing service and work autonomously within a supportive team. In this role you will support the wider CHC team by case managing highly complex individuals and their families/carers helping them to access the most appropriate care to support their needs. The role can be challenging and therefore rewarding as well diverse day to day. The role includes case managing a variety of patients i.e. physical health, learning disability, mental health, ABI, etc. in a variety of settings. You will work across the ICB footprint visiting patients in a variety of settings including their own homes as well as meetings on MS Teams.
