We have an exciting opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP Foston Hall, a closed category women's Prison and Young Offenders Institution, located in the village of Foston in Derbyshire, England.

Hours
37.5 hours a week working ideally 3 long days a week from 7am-7.30pm or would consider someone who wants Monday to Friday 8-4pm. Some weekend work required but only very occasional.
Salary
This position is full time, you'll receive an annual salary up to £47,466 per annum FTE - depending on experience.

What you'll be doing
As a Long Term Conditions Nurse, your responsibilities will vary. You will:
* Be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
* Identify immediate health needs including any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing.
* A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education.
* Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections.
Qualifications required
* Registered Nurse- General with relevant NMC Registration
* Must have experience of managing long term conditions
* Prescribing would be an advantage
* Educated to degree level
* Knowledge of Evidence Based Practice and Clinical Governance
* Experience of assessing the needs of and working with individuals with complex health and social care needs.
Experience in a prison is not essential.
