Job summary Following a re-structure of our ways of working a fantastic opportunity has arisen for proficient and highly motivated registered clinicians (Nurse or AHP) to join our existing Continuing Healthcare team. This role require a dynamic individual who is an effective communicator, is forward thinking and able to perform both independently and as part of a team. The ideal candidate will have the ability to co-ordinate and manage competing priorities, good organisational skills, time management and a solid understanding of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care.
