Job summary SHaR PCN is a group of seven doctor's surgeries and medical centres that work closely together as part of a network. It employs health professionals including clinical pharmacists, occupational therapists, first contact physiotherapists, paramedics, urgent care practitioners, health and wellbeing coaches, first contact mental health practitioners, specialist care co-ordinators (e.g. dementia and frailty) and social prescribing link workers. Working as part of multi-disciplinary teams, these clinical and non-clinical health professionals provide tailored care for patients allowing GPs to focus more on patients with complex needs. Whilst working for and across the SHaR PCN network, the host practice that will manage the role on a day to day basis is Pickering Medical Practice. Main duties of the job High quality data collection, co-ordination and monitoring is essential in focussing and improving the quality of care to patients and tackling health inequalities. The role will provide data reports, data quality and assurance support to the South Hambleton and Ryedale Primary Care network (SHaR PCN) through monitoring the accuracy of the data in the clinical systems as well as providing audit support and activity reporting. The role will also include supporting digital initiatives and responsibilities for the co-ordination of patient journeys through primary care with a particular emphasis on the digital journey.
