Main Duties of the Job
As a very brief overview, your role will be to work closely with acute Trusts to support discharge to assess pathways, completing all reviews and assessments in line with the services key performance indicators.
You will:
1. Deliver optimum holistic care to service users identified for NHS healthcare funding.
2. Be actively involved in ensuring that service users meet the eligibility criteria for NHS Continuing Healthcare.
3. Ensure service users are reviewed in line with national and local policies and procedures, as well as those eligible for Section 117.
4. Assess, commission, plan, and procure packages of care for service users in line with the AACC Commissioning Policy.

Job Responsibilities
This will be varied and will adapt over time as both the ICB/S develops. However, the postholder is expected to fulfil the following key functions:
1. Play a key role in supporting the Continuing Healthcare service across the region.
2. Working within the remit of the National Framework for Continuing Healthcare and funded-Nursing Care and the Mental Health Act, the post holder will deliver an effective and competent level of service and consistently deliver a person-centred service.
3. Be responsible for assessing individuals, including those with a functional mental health condition, ensuring the National Framework for NHS Continuing Healthcare and Funded-Nursing Care and Mental Health Act are implemented and complied with according to the principles and processes.
4. This includes management of packages of care, ensuring these are commissioned to meet identified health outcomes.
Key Working Relationships
The post holder will have regular contact with a wide range of staff at all levels within the ICB/S, with senior and other representatives of local stakeholders, as well as with patients and their representatives. Some of the key direct relationships include:
* Cheshire and Merseyside ICB/S executive teams
* Cheshire and Merseyside ICB leadership teams
* Line managers
* Colleagues within NHSE/I
* Senior leaders across Cheshire and Merseyside, NHS, local authorities and other third sector organisations
Main Duties and Responsibilities
1. Ensure assessments are undertaken in line with the National Framework for Continuing Healthcare, Mental Health Act and funded-Nursing Care and submitted to the ICB within the services key performance indicators.
2. Undertake section 117 after care funding applications.
3. Timely review of patients including regular CPAs.
4. Commissioning of appropriate 117 aftercare packages.
5. Complete all review assessments in line with the service key performance.
6. Work closely with acute hospitals to support discharge to assess pathways.
7. Accurately input information onto departmental databases and tools used in Continuing Healthcare adhering to NMC standards for record keeping.
8. Assist and support individuals and representatives in relation to the Continuing Healthcare process.
9. Commission appropriate packages of care for individuals eligible for CHC or other NHS funded care, in line with the Commissioning Policy.
10. Case management and ongoing review of individuals eligible for CHC or other NHS funded care in accordance with the National Framework for Continuing Healthcare, the Mental Health Act and NHS-Funded Nursing Care.
11. Provide assurance regarding quality of individual packages of care by undertaking reviews as required and reporting quality/safeguarding concerns through the agreed reporting route in a timely manner; Undertake urgent reviews of individuals where quality and safety concerns have been identified.
12. Ensuring data systems are kept up to date and maintained in line with standard operating policy and procedures.
13. Management of complaints/disputes.
14. To prepare and collate appropriate documentation to enable the level of NHS funding to be determined within the parameters of the National Framework for the Mental Health Act, Continuing Health Care and NHS Funded Nursing Care.
15. Ensure PHBs are actively promoted with service users and their representatives.
16. Interpret regional and national guidance including the Who Pays Guidance.
17. To process urgent fast track referrals and, where appropriate, procure packages of care in a timely manner for individuals who are receiving or require end of life care.
18. Support young adults transitioning from child to adult services, awareness of policies pertaining to this assessment.
19. Maintaining constructive relationships with a broad range of internal and external stakeholders.
20. Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.
21. Maintaining up-to-date knowledge of legislation, policies and guidance.
22. To have an understanding of the Mental Capacity Act and Deprivation of Liberty of Safeguards; completing relevant documentation for Court as appropriate.
23. To maintain professional confidentiality in all matters relating to patient care.
24. Promoting transparency and a person-centred approach with patients, families and representatives.
25. To actively promote and implement personal health budgets.
26. To participate in audit within the service.
27. To participate in the delivery of training programmes for staff across health.
28. To actively promote communication and partnership working between the statutory, independent and voluntary sectors.
29. Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service.
30. To contribute to team function by sharing own clinical skills/expertise and knowledge.
31. To identify any training needs within the independent and statutory services and ensure this information is fed back appropriately.
32. To line manage staff as requested by the Line Manager ensuring HR policies and procedures are followed.
33. To ensure any staff that are line managed by the postholder receive appropriate supervision.
34. All staff are expected to adhere and complete the NHS Continuing Healthcare competency framework on an annual basis.

* Educated to degree level or equivalent level of qualification or experience in working at a similar level in specialist area.
* Nursing qualification with current registration.

* Experience of working in a health care environment.

* Experience of partnership working with other agencies.
