We operate Federation-led services including Respiratory and Spirometry hubs; Community Cardiology; Community Dermatology; Community Diabetes service; Access E-hub and Enhanced Access and we support PCNs with managing their NWL single offer contract of services and the National Network DES contract, including ARRS roles. Requirements ROLE PURPOSE · We run a range of services and our ANPs work across 3 key services: - Our eHub is a digital same-day service for our 33 practices. ANPs speak to and possibly see patients assigned to them by our senior clinical decision maker who triages all the e-consultations we receive. - We run an Extended Access service in the afternoons and evenings of Mon-Fri, and from 9-4 on Saturdays. - We run research clinics, where we see and screen patients who are eligible for our studies. - Furthermore, we employ GP assistants and Nurse Associates and ANPs will be involved in the training and supervision of these staff. · The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice and other clinical settings. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. KEY TASKS AND RESPONSIBILITIES Clinical · Working across the following specialties; eHub, Enhanced Access and research · Triage urgent care requests if required, consulting with patients or directing them appropriately. · Provide advanced nursing care to patients, including assessment, diagnosis, treatment, and follow-up. Conduct holistic assessments on patients and ensure appropriate care and action plans based on their needs are clearly recorded and documented. Undertake regular patient reviews. Plan patient discharges safely and effectively, including use of independent prescribing in respect of providing medications on discharge. Following assessment where appropriate/necessary and with the patients’ consent refer appropriately to other services and agencies. · Perform and interpret diagnostic tests, such as laboratory and radiological tests. · Prescribe and administer medications in accordance with the law and established protocols. · Collaborate with the healthcare team, including physicians, nurses, and allied health professionals, to ensure comprehensive patient care. · Maintain accurate patient records, including medical histories, treatment plans, and progress notes. · Educate patients and their families about their health conditions and treatment options. · Stay up-to-date on the latest medical research and guidelines relevant to the clinical specialty. · Maintain compliance with all relevant clinical standards and regulations. · Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports. · Promote health and well-being using health promotion, health education, screening, and therapeutic communication skills (i.e. smoking cessation, weight management, alcohol misuse). · Refer patients directly to other services/agencies as appropriate.
