The Inreach (hospital social work team) is looking for an enthusiastic, motivated Social Worker to join our busy team facilitating discharges from hospital either into long term care or back to their own homes with support. You will undertake a range of tasks including assessments, reviews, planning for packages of care, AWI meetings and providing support to service users and carers at often a crisis point in their lives.
The team provide a service to all service users over 16 who live in Midlothian and need support to leave hospital. We cover all hospitals in the area including Edinburgh Royal Infirmary, Western general and Midlothian community hospital. We work closely with our NHS colleagues in hospital and throughout the Midlothian health and social care partnership.
Skills
* The successful candidate will be based in Bonnyrigg health centre but will be expected to travel for meetings and to complete assessments at the hospitals in Lothian area.
* Accurate case recording and report writing and clear risk assessment skills
* Ability to consider all available information to inform assessment, agree outcomes and develop action plans.
* Excellent communication and Interpersonal skills
* Be able to be adaptable and to be able to work in a fast-paced environment
* Be able to work well as part of a team and act on own initiative and prioritise your workload
* Full (valid) driving licence is preferable
KNOWLEDGE
A degree in Social Work and registration with the SSSC as a Social Worker is essential a Diploma in Social Work with SSSC registration as a social worker is an acceptable alternative together with a Mental Health Officer academic qualification.

Please note that it is Midlothian Councils policy that successful applicants will be appointed to the first point on the salary scale.
Based on you working 36 hours over 52 weeks per year the minimum actual salary for this position is £41,802.34 per annum.
