Our Vision is to provide the best mental health, learning disability, autism and community based services for the populations we serve.
As an integrated mental health, learning disability and community Trust, Lancashire and South Cumbria NHS Foundation Trust provides a range of services including:
1. Primary and secondary mental health care for children and adults including specialist inpatient child and adolescent mental health provision, perinatal mental health, forensic services including low and medium secure care.
2. Specialist community support for children and adults with learning disabilities and autism, including intensive support.
3. Community physical health and well-being services for children and adults.
The Trust was first established in 2002 and employs approximately 7,000 staff who provide care from more than 400 sites. The organisation offers opportunities for medics, mental health and general nurses, allied health professionals, psychology, administration and clerical staff, apprentices and volunteers.
Lancashire and South Cumbria NHS Foundation Trust supports flexible working. We believe that a positive work/life balance benefits NHS employees through improved health and wellbeing. We would consider working patterns such as term time, part time, compressed hours, home/remote working, and flexi-time.
Are you a qualified and registered Nurse, Social Worker or Occupational Therapist seeking a new and exciting challenge?
We are recruiting a band 6 practitioner to help support the ongoing development of our Discharge Team working with service users whose hospital care is being delivered outside of LSCFT. The post holder will be expected to meet service delivery working Monday - Friday, 0900-1700.
These posts will be based at The Harbour, Blackpool.
The Fylde Coast currently has out of area providers giving care to some service users. Given recent research, it is important that localities are supporting admissions further away from home to reduce their occurrence. The candidate will have the knowledge, skills and drive to support Fylde service users through their admissions to out of area hospitals and The Harbour.
This is an interesting development opportunity for experienced practitioners to join a team refining the discharge pathway for patients, particularly those with complex needs requiring funded packages of care.
Main duties of the job
You will provide clinical management and co-ordination of complex mental health discharge pathways working collaboratively within a wider multi-organisational team as part of co-produced service developments. The post holder will require a working knowledge of commissioning processes.
The post holder will use their experience and knowledge of specialist discharge pathways and out of hospital provision to influence discharge decisions; promoting alternatives to restricted environments and working with the patient and the clinical team to increase confidence in the combined impact of multi-agency discharge support, balancing risk and responsibility. The post is largely ward-based supporting the ward Multi-Disciplinary Teams around effective discharge planning. Service need is across a number of our LCFT wards and the post holder may be required to travel across LCFT hospital sites.
You will have a role in contributing to and reviewing the discharge plans of patients to ensure that the patient is progressing (including rehabilitation) to the least restrictive environment that will meet their needs for safety and service quality. You will be responsible for making recommendations for patients who are suitable for transfer to a less restrictive environment and will support the Care Coordinator with this process.
