Planning and Organisational Duties To assist the therapy service in the assessment and treatment of patients for diverse range of clinical conditions and presentations. Utilise judgement skills, the post holder will be required to assess and evaluate patient conditions and within parameters set by the Occupational Therapists and Physiotherapists to make relevant alterations to existing treatments/equipment or progress a patients rehabilitation programme onto a different stage within agreed protocol based clinical care. This could be done without prior consultation with the therapist and may include discontinuing rehabilitation program if judged unsafe or modifying activities or equipment. Conducting initial interviews to gather information prioritise and analyse for the therapists. Liaise closely with supervising therapist, updating and reporting any significant changes in the patients condition, to facilitate modification of the treatment programme to the therapist. Promote safe moving and handling practices, according to patients needs within the manual handling policy. Carry a dedicated caseload of patients, under supervision of qualified staff. To identify and contribute to person-centred goals identified with the patient, evaluating progression and recognising changes required with qualified clinicians. To engage patients in therapeutic activities to promote independence, wellbeing and social inclusion, adjusting interventions accordingly with the support of qualified staff. To provide support, advice and education to patients, carers and others in the rehabilitation and self-management of the person concerned. Including the delivery of health promotion strategies. To undertake a range of discipline specific assessments, (including BP monitoring), (weight management). Recording and evaluating outcome measures with qualified support. To assess and prescribe equipment, minor adaptations and refer to other services for specialist adaptations. To clearly document patient interventions in case notes, following organisational policies and local standards. Ensuring patient contact statistics and discharges are completed in a timely fashion. To maintain accurate patient records in accordance with trust guidelines for records and records keeping, complying with the Data Protection Act. Organise and prioritise own allocated workload, incorporating time management and forward planning, thus acknowledging the need to meet deadlines. Understand and apply risk and capacity assessments as appropriate, including acting in the best interest of the service user with support from qualified staff. Clinical completion of an episode of care, appropriate communication of discharge within agreed protocols. Attend and participate in meetings to discuss patient progress, future treatment plans and diary planning for the week ahead. By utilising knowledge of the wider community services, support professional colleagues to access and refer to the most appropriate service to meet patient needs. Manage time and caseload pressure in a rapidly changing environment, using appropriate prioritisation. Demonstrate flexibility as the service requires. Undertake any other professional duty as required within the service, in accordance with training, and experience following consultation with the trained member of the team. To work professionally and in a non-discriminatory manner, be legally responsible and accountable for all aspects of the work undertaken. Deal with challenging behavioural situations including verbal aggression or distress, seeking support as necessary. Promote and educate others regarding the role of the Intermediate rehabilitation teams, and the importance of occupation and function in relation to health and wellbeing To participate in the non-clinical aspect of team working, such as housekeeping duties, equipment ordering and monitoring and documentation tasks. Communications and Key Working Relationships To have excellent communication skills to enable effective and professional communication with team members, other clinical staff, patients and their families. To demonstrate an ability to communicate through email and telephone correspondence effectively and professionally, video calls and face to face conversations. To establish and maintain appropriate clinical relationships with patients to assist in the delivery of effective patient centred care, using a range of techniques to overcome communication barriers. Participate in case conferences/meetings and liaise with other disciplines to achieve comprehensive, effective and holistic ongoing care management planning for the patient. Demonstrate the ability to communicate information, sometimes of a complex and sensitive nature and respond appropriately to patients, carers and staff. Maintain accurate records according to the Trusts standards and ensure that all reports are completed and sent appropriately. Ensure timely and effective communication with the qualified staff or Team Leader on all team matters. Responsibility for Training Participate in on going professional development activities. Keep up to date with clinical developments implementing change to clinical practice accordingly, under the supervision of the qualified clinician. Undertake regular clinical supervision sessions in line with Trust Policy. To be accountable for the completion of own mandatory training, in line with trust policy. Actively participate in your own personal development plan with your supervisor in order to develop the skills and knowledge required to carry out the responsibilities set out in the job description.
