Job Summary
We are looking to recruit an enthusiastic individual to join us as a key member of our multidisciplinary team which includes a Consultant Practitioner, Advanced Clinical Practitioners, Registered Nurses, Occupational Therapists, Physiotherapists and Paramedic Practitioners, and Health Care Support Workers. The team provides an urgent response to patients who are experiencing a health crisis with the aim to support them in their own homes rather than being admitted to hospital. You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards.
The post holder will provide clinical expertise, by providing assessment, advice, and information according to clinical guidance through telephone triage, and face-to-face assessments. They will provide assessment and care planning including history taking, physical assessment and treatment planning, without direct supervision. Provide complex case management using extended skills where appropriately trained to avoid hospital admission and manage complex clinical needs in the community setting.
The post holder will be expected to work flexibly with 2 weeks clinical triage and 2 weeks patient facing on a 4 weekly pattern including 1 in 4 weekends.
Main Duties of the Job
1. Provide clinical expertise, by providing assessment, advice, and information according to clinical guidance through telephone triage and face-to-face assessment.
2. Be a central contact for the UCR team, using initiative to triage phone calls and emails ensuring appropriate timely action is taken.
3. Work closely with the Triage Support, using judgement and experience to decide which calls and documents can be delegated to them.
4. Support the coordination on the daily team handover.
5. Coordinate appointments within a very busy team.
6. Provide assessment and care planning including history taking, physical assessment and treatment planning, without direct supervision.
7. Face to face assessment and treatments of frail patients who are acutely unwell.
8. Be willing to undertake the 'deteriorating patient' academic unit at level 6/7 depending on current qualifications at university, organised and funded by SHFT.

* Appropriate professional qualification e.g. RGN, Occupational Therapist, Physiotherapist or Paramedic, with a minimum of 2 years post qualification experience.
* Professional UK registration e.g. NMC / HCPC.
* Evidence of Continued Professional Development (CPD) clearly recorded for professional profile.
Experience

* Computer Literate in both data entry and communicating via IT - emails and reports.
* Full driving license and access to a car.
* Ability to meet the travel requirements of the post which will include travel to other areas of the trust for training and home visits to patients' place of residence.
