Specialist Community Public Health Practitioner – Health Visitor

Whether you are an experienced health visitor or newly qualified, we will ensure you receive individualised and effective support from both your health visiting line manager and colleagues within your team.
You will work within the West Berkshire area to deliver core and targeted services to children aged 0-5 and their families. You will require excellent interpersonal skills, the ability to work in partnership with other teams and agencies, along with great communication skills and a passion for working with children and families.
A commitment to team working, flexibility and working with partners is essential, as you will collaborate with Children's Centres in the implementation of the Healthy Child Programme underpinned by the Solihull Approach model of working.
If successful your base will be West Berkshire Community Hospital (Thatcham); however your role will require you to visit homes across Berkshire (mileage will be compensated).
Main duties of the job
1. Develop and implement outcome focused care plans and evaluate care given, in partnership with clients.
2. Provide support to families using the Solihull approach model in behaviour management, parenting skills and delivering programmes of care to vulnerable families.
3. Develop and facilitate Health Visiting drop-in clinics, health promotion activities and groups.

Job responsibilities

1. You must be a Registered Specialist Community Public Health Nurse (Health Visiting) active on the NMC register
2. You will be a Non-medical prescriber
3. You will hold a valid UK driver's license with access to a vehicle

Education/Qualifications/Training
* Registered Public Health Nurse (Health Visiting) active on the NMC register
* Non-medical prescriber
* S.L.I.C.E/ENB 998 or equivalent
Continuous Professional Development
