Futures+ is a unique and important team within East Riding Adult Specialist Services. We work in accordance with the Care Act 2014. We provide assessments, care & support planning and reviews for adults with learning disability and/or autism. Our service users require a high level of support in many aspects of their daily life and rely on a wide range of health and social care services.
Futures+ is based at County Hall, Beverley, with opportunities for hybrid working. We are a friendly and supportive team with diverse experiences, knowledge, and skills. We are looking for one confident, competent, motivated, and qualified social work practitioner to join our team. Strong interpersonal and organisational skills are essential. The role can be demanding, so you need to be a team player with a can-do attitude.
Within Futures+, we have two hubs: Preparing for Adulthood (PfA) and Community Inclusion (CIH). We are recruiting for one part-time social worker post (18.5 hours a week) for the PfA Hub, which supports children aged 14 or above with complex needs transitioning from children’s services to adult services. Service users will transfer from the PfA hub to the CIH hub either when their education ends or when they turn twenty-five years old.
In Futures+, we embed a person-centred and strength-based approach when working with our service users, their carers, and/or families to carry out Care Act assessments, formulate care and support plans, put together panel applications, and conduct reviews. We work collaboratively with colleagues from other agencies and contribute to multi-agency meetings. For a few days a month, you will take on the role of duty officer, answering enquiries and dealing with urgent requests from the public or other professionals. While on duty, you will be supported by a senior social worker and work alongside another team member. We pride ourselves on our strong team spirit and have an induction program to support new staff members.
You must be a registered social worker with sound knowledge of relevant legislation and guidance, including the Care Act 2014, the Mental Capacity Act 2005, and the Human Rights Act 1998. A strong understanding of person-centred and strength-based approaches is required, along with experience in multi-agency working.
Depending on your experience, you will be expected to undertake supervisions of Enabling and Transitioning Officers.
