We wish to recruit a three day Speciality Doctor post to develop our Same Day Emergency Care services in Acute Frailty.
This post is part of transformational change within the department. They are part of a strategy, approved by the Trust board, which will radically change the depth and breadth of services offered to older people across the Trust footprint.

Acute Frailty Unit
We are proud of our Acute Frailty Unit that continues to gain momentum in providing ambulatory care to frail older patients in crisis. We have a motivated multidisciplinary team delivering urgent Comprehensive Geriatric Assessment (CGA) along with excellent access to diagnostics and specialty liaison. Our most recent data has shown this has a positive impact on both same day discharge as well as shorter length of stay for frail older patients when admitted.
Main duties of the job
The primary role will be to support the Acute Frailty Team across the Urgent Care setting. This will require working out of ED or the AFU, and may also involve liaison with MAU or AAU where required, within a multi-disciplinary team to provide acute Comprehensive Geriatric Assessment and identify care needs for discharge. This will require excellent communication and team-working skills, being able to liaise with nursing staff, therapists and discharge team.
The Speciality Doctor will be expected to work alongside the responsible Consultant Geriatrician to provide continuity of care, rapid assessment of presenting conditions, development of management plans and diagnoses and communicate these effectively to the patient and their family members. Whilst the named Consultant Geriatrician for the day will retain overall responsibility for patient care, the Specialty Doctor is expected to work independently wherever possible to plan and deliver patient care including making discharge decisions.
The successful candidate will be expected to take part in audit, clinical governance reviews, including morbidity and mortality meetings and participate in the departmental educational programme. They will also be expected to contribute to training of undergraduate medical and PA students. The department welcomes and would support post holders in attaining further post-graduate diplomas in Geriatric Medicine and Education.

Essential
1. MB ChB or equivalent
2. Full registration with the GMC
3. MRCP/MRCGP or Diploma of Geriatric Medicine
4. 2 years previous NHS experience
Desirable
1. Higher degree (MD or PhD)
2. ALS
Knowledge & experience
Essential
1. Ability to work independently
2. Ability to undertake Comprehensive Geriatric assessment
3. Ability to manage patients at End of Life and in Last Days of Life
4. Ability to manage patients dementia, delirium and with challenging behaviour
5. Good time management and understanding of patient flow within Urgent Care setting

1. Demonstrate evidence of wide based medical knowledge

3. Clinical/Educational Supervisor Training
4. Extensive experience of Geriatric Medicine
