Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for an Advanced Practitioner within the Safeguarding & Support Team to work full time based in Northamptonshire.
The salary for this permanent Advanced Practitioner job is up to £54,735 per annum.
Main duties:
1. Provide professional support, leadership, and line management/supervision to staff that form part of an integrated multi-disciplinary team that includes qualified and unqualified social workers, occupational therapists, independent living assessors, rehabilitation workers, and related support services.
2. Provide clear leadership to staff responsible for providing assessment and care management services, ensuring decisions determining a service user’s eligibility are assessed based on needs and risks, including managing the duty and review systems.
3. Manage a high-quality front-line duty service, ensuring appropriate access to the public, referring agents, and other professionals, and ensuring that emergency situations are dealt with in a timely manner.
4. Hold a small, complex caseload, proactively case manage and undertake proportionate assessments, and effectively risk manage complex situations with people who use our support and services.
5. Complete support planning with adults and carers to ensure that outcomes identified at assessment are met through the provision of support, equipment, adaptations, and enabling programmes.
6. Assist Team Managers in the day-to-day operational management of the assessment and care management service for young people who are the responsibility of the council.
7. Supervise, manage performance, develop, and appraise members of the service as outlined in Departmental guidelines.
8. Contribute to performance monitoring of the staff group, the service, and the Department regularly to ensure that performance indicators are being met.
9. Authorise care packages up to agreed limits and screen for suitability of residential/nursing home placements where necessary.
10. Establish, challenge, and review care packages, support plans, minor works, and equipment, exploring all income sources in relation to care packages and sourcing cost-effective resources.
11. Work with others to influence and shape the local environment, promote social inclusion and accessibility, and ensure that identified needs are understood individually, by locality, and fed into strategic commissioning.
12. Deputise for the Team Manager in their absence as required and carry out any other duties at an equivalent level as directed by the Team Manager, including taking part in management meetings, training, and other developmental duties as required, and chairing regular team meetings on a rotational basis.
Requirements of this Advanced Practitioner job:
1. A social Work Qualification or equivalent.
2. Understanding of relevant legislation.
3. Continual professional development.
4. Social Work England registered.
