CHS Community Support Services work with vulnerable people who are homeless or at risk of homelessness, to promote independent living skills. The services also work with families/relatives to prevent homelessness and they support people to sustain their tenancies where they are in independent accommodation.
What You’ll do :
In this varied role, you will provide the highest quality support to young people aged 16 – 25 to enable them to achieve their personal aspirations and live as independently as possible.
You will do this by working closely with other agencies; keyworking an allocation of service users, meeting them regularly to develop, progress and review their individual action plan, to support them achieving their goals and independent living skills. This includes coping strategies, maintaining their physical and mental wellbeing, tenancy sustainment, obtaining/maintaining employment, education or training and social activities.
You will also prepare accurate written and electronic records of daily events, support sessions, action plans, Rickter Assessments and support reviews etc.

Able to work creatively to meet the needs of a diverse range of young people.

Experience advising and supporting young people.

Able to maintain firm boundaries, whilst also working flexibly to meet individual needs.

Good communication skills – verbal, written and listening.

Strong IT and office skills, and the ability to maintain office records and write short reports.

Able to work without close supervision, and to make own decisions and prioritise tasks when necessary.

Able to manage challenging situations when alone.
*
A full UK driving licence, use of a car for work purposes and business insurance is essential.
