Role Description
This is a full-time role for a Care Coordinator, based on-site in Newcastle Upon Tyne. The Care Coordinator will be responsible for managing care schedules, coordinating care services, ensuring efficient communication with clients and caregivers, and delivering high-quality customer service. The role also includes conducting assessments, creating care plans, and ensuring compliance with relevant regulations and guidelines.
Qualifications
* Strong skills in Care Coordination and Case Management
* Excellent Communication skills
* Experience in Home Care and Customer Service
* Ability to work effectively on-site in Newcastle Upon Tyne
* Knowledge of care industry regulations and best practices
* Relevant certifications or training in health and social care are a plus
