The Role
An opportunity has arisen for a Community Assessment Officer (CAO) post within the Discharge to Assess function of The Independent Living Service. The Independent Living Service has 26 staff made up of Occupational Therapists, Disability Officer and Community Assessment Officers and includes the Sensory Team and Telecare Team. The aim of the post is to aid with discharge facilitation and community follow up. The post will sit alongside Community Therapy and the Intermediate Tier Social Work team and looks to support people upon discharge who could be at possible risk of readmission to hospital without support. The CAO will be supported to be “trusted assessors” for equipment and have basic moving and handling training to support therapy if required and have access to Reablement if required.
This post offers an opportunity to contribute to the ongoing development of innovative ways of working across health and social care to improve the experience of service users and carers and to promote well-being and independence.
The ability to develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in inter-disciplinary team working.
The post is subject to enhanced clearance from the Disclosure and Barring Service.
