As a Frailty Care Coordinator, the successful applicant will work closely with clinical teams helping to provide optimal care for frail patients. The role involves some patient contact by telephone and in person, and a positive, respectful, and compassionate nature is necessary when delivering a high-quality, safe and effective service with the highest standards of confidentiality.
The Care Coordinator will be provided with all necessary training to fulfill the project to its full potential.

Main duties of the job
DHC is looking for a motivated and compassionate person who either has, or is working towards, a healthcare certificate, and who enjoys patient contact and the challenge of providing great quality care. The role involves some home visits and some clinical care, and you will work with a variety of clinical and non-clinical staff to ensure that frail patients receive the most joined-up and appropriate care possible.
You will use data searches to seek out patients for care and support with innovating pathways for frail patients, along with a team of healthcare professionals. You will also liaise with care homes in the area to optimize the relationship with primary care for patient benefit.

Job responsibilities
The Frailty Care Co-ordinator will have a pivotal administrative role in working with the practices and the wider primary/community care team to ensure frail patients receive appropriate interventions to support them to live as independently as possible. This will involve supporting the coordination of a range of clinicians and social care. It will also involve working with individual patients to identify, then facilitate any support they would benefit from being delivered by the voluntary sector or health services.

Qualifications
* Educated to at least GCSE level or equivalent
* Possession of or working towards a Care Certificate.
Experience
* Proven experience of managing a complex administrative role
* Experience working with the general public in a similar role
* Experience of working in a similar healthcare organisation or in the leisure industry/voluntary sector or in education or similar
Skills and Aptitudes
* Excellent administrative skills working under own initiative at times
* Strong, confident IT skills - able to use relevant Office packages - Word, Excel, Databases and Outlook for e-mail and calendar and Databases with patient information
* Comfortable working in often fast-moving and changing situations, with ability to adapt to the changing needs of the service
